As far as I know its a push button shifter, the same setup that Brian Kelly has

How does he get on and off? I've seen Brian get on and he uses his good leg to support the bike while he gets is other foot/leg into place.

Probably with any challenge it would take a good amount of muscle memory and getting used to working with what you've got. Before I bought my GSXR, I sat on a bunch of bikes at the dealership and my primary concern was the height issue. A couple of people reassured me at the dealership that it's really not a problem, there is a guy who works there that is a little person and when he has to stop, he literally jumps off his bike, waits, and when it's time to go again he jumps back on. I didn't get the chance to meet him but I've seen him, he's uber in my book. I'd definitely like to see how this vet does it, very cool.

I think I know who you are talking about. Is he the really short guy that works or worked at Honda Suzuki North in Carrollton that's only like maybe 4'10 in his tallest boots?
